All amounts recovered under the penalties herein provided shall be appro

priated to a special fund for the carrying out the objects of this law.

SEC. 8.-BOOKS OF BIRTHS AND DEATHS.

The County Clerks of the several counties in the State shall be required to keep separate books for the registration of the names, and the post office address of Physicians and Accouchers, for births, for marriages, and for deaths. Said books shall always be open to inspection without fee; and said County Clerks shall be required to render a full and complete report of all births, marriages and deaths to the Secretary of the Board of Health annually, and at such other times as the Board may direct.

SEC. 9.-FORMS.

It shall be the duty of the Board of Health to prepare such forms for the record of births, marriages and deaths as they may deem proper; the said forms to be furnished by the Secretary of said Board, to the County Clerks of the several Counties, whose duty it shall be to fu:nish them to such persons as are herein required to make reports.

SEC. 10.-MEETINGS OF THE BOARD.

The first meeting of the Board shall be within fifteen days after their appointment, and thereafter in Jannary and June of each year, and at such other times as the Board shall deem expedient. The meeting in January of each year shall be in Springfield. A majority shall constitute a quorum. They shall choose one of their number to be President, and they may adopt rules and by-laws for their government, subject to the provisions of this Act.

SEC. 11.-OFFICERS-COMPENSATION.

They shall elect a Secretary, who shall perform the duties prescribed by the Board, and by this Act; he shall receive a salary which shall be fixed by the Board; he shall also receive his traveling and other expenses incurred in the performance of his official duties. The other members of the Board shall receive no compensation for their services, but their traveling and other expenses, while employed on business of the Board, shall be paid. The President of the Board shall quarterly certify the amount due the Secretary, and on presentation of his certifi cate, the Auditor of State shall draw his warrant on the Treasurer for the amount.

SEC. 12.-ANNUAL REPORTS.

It shall be the duty of the Board of Health to make an Annual Report through their Secretary, or otherwise, in writing to the Governor of this State, on or before the first day of January of each year, and such report shall include so much of the proceedings of the Board, and such information concerning vital statistics; such knowledge respecting diseases, and such instruction on the subject of Hygiene as may be thought useful by the Board, for dissemination among the people, with such suggestions as to legislative action as they may deem necessary.

SEC. 13.-APPROPRIATIONS.

The sum of five thousand dollars ($5,000), or so much thereof as may be necessary, is hereby appropriated to pay the salary of the Secretary, meet the contingent expenses of the office of the Secretary, and the expenses of the Board, and all costs for printing, which, together, shall not exceed the sum appropriated; said expenses shall be certified and paid in the same manner as that of the Secretary.

SEC. 14.-ROOMS.

The Secretary of State shall provide rooms suitable for the meetings of the Board, and office room for the Secretary,-(Laws of 1877, pp. 208-210.)

Certificates have been issued to date as follows:

To graduates, 3,786.

To non-graduates, based upon affidavits of ten or more years' practice in the State previous to July 1st, 1877, and recommendations of reputable physicians, 885. To non-graduates who have passed the examinations of the Board, 136.

Total, 4,807.

Of this number 750 live in Cook Co, 3,978 in other counties of the State, and 79 in adjoining States, whose practice extends into Illinois.

Certificates have been issued to 277 midwives, authorizing them to practice midwifery only. Of these, 240 were based on affidavits of ten or more years' practice in Illinois previous to July 1st, 1877, and recommendation of reputable physicians, or certificates of graduation from reputable schools of midwifery. And 37 passed examination by the Board. Eight were examined who failed to pass. There are about 600 midwives in the State, of which more than 200 are in Cook County. The Board has entirely suppressed the schools for midwives in this State.

One hundred applications without affidavits were rejected.

One hundred and sixty-eight affidavits are on file in the office of the Board, from those who have practiced in this State for ten or more years previous to July 1st, 1877, and who do not apply for a certificate. While it is not obligatory for this class to take a certificate, still it is much better for all who can to do so.

Thus far the Board has had occasion to revoke eight of the certificates issued. In 41 instances doctors who had been licensed by the Board, and who afterward were guilty of unprofessional advertising, on being notified by the Board, withdrew the objectionable publications.

When the Board began operations there were about 7,000 practicing physicians in the State, of which about 3,000 were non-graduates. Over 1,300 have either left the State or ceased to practice.

The registration of births and deaths is being satisfactorily carried on, as contemplated by the provisions of the law, in nearly all the counties of the State, but in a few it is inefficiently done.

Considering the length of time the Board has been operating, the extent of the work in an entirely new field, and the insufficient appropriation, the results, although as yet incomplete, are beyond expectation.
